Before You Request (Checklist)

To schedule your appointment, please have ready:

  • Names of your primary care and/or referring physicians
  • Active health insurance details: plan, ID/subscriber number, group number, guarantor’s name
  • Your demographic information: name, date of birth, address (see forms for more info)
  • Scheduling your new visit may take 10–20 minutes by phone.
  • Please check your insurance network status.
  • Uninsured? A $200 payment will be due at check-in.
