How to Sign-Up For Established Patients:
Patients who wish to join our Breeze portal must call our office to receive an invitation to the portal. We will need the patient/child's legal name, DOB, phone number, and email address that we have on file.
*Please Note*
- Each patient must have their own email address.
- The information entered must match the information we have on the patient’s file. If you are unable to connect, please call our office as we may have incorrect or missing information.
- For families with multiple established patients, each patient must have a portal account created. Then, all the accounts can be merged and accessed under one patient’s log-in credentials.
- Patients may use phones, computers or tablets to complete. Iphone users may use the website or download the app. Android users will need to download the app.

The Breeze support line can assist with password & email changes and other technical issues. When a patient becomes a legal adult at the age of 19 per the State of Nebraska, the patient may call this line to retrieve ownership of their Breeze account.
